Employee Request for Disability or Pregnancy-Related Accommodation

Missouri Western State University is committed to providing an accessible and supportive environment for employees with disabilities. The university provides reasonable accommodations to qualified job applicants and employees (including teaching and research faculty, classified and exempt professional staff, and student employees) with a disability in order to enable the employee to perform the essential functions of her or her position.

The university provides reasonable accommodations to employees experiencing conditions related to pregnancy and childbirth.

An employee who would like to request a reasonable accommodation due to disability or pregnancy, in order to perform the essential functions of their position, may make a request by submitting an Employee Accommodation Request Form. If an employee makes a request to the employee’s supervisor, the supervisor should refer the employee to the Employee Accommodation Request Form and should also notify the Director of Human Resources/Equal Opportunity Officer of the employee’s inquiry.

Accommodation is an interactive process between the university and the employee. Requests for accommodations are reviewed by the Director of Human Resources/Equal Opportunity Officer or designee as soon as practicable, on a case-by-case basis, and circumstances may require the submission of additional information or an independent examination or evaluation (at MWSU’s expense) for the university to make the determination.  Employees may be asked to provide a completed Health Care Provider Statement. Medical information will be kept confidential and maintained separately from your employee personnel file. MWSU will not be required to provide accommodations without appropriate documentation when requested.

NOTE: Not all illnesses, conditions, and/or impairments are considered a “disability” under university policy and/or applicable state and federal laws. Prior to implementation of any reasonable accommodation, the Director of Human Resources/Equal Opportunity Officer will utilize applicable ADA laws and guidelines to determine if a disability exists and/or if the individual can be qualified as a person with a disability.
