Medical Records

All new students are required to complete and return required information as soon as possible.  Also, for student’s safety, well-being and protection while at the university, a few immunization or preventative injections/shots are REQUIRED for all NEW students, staff and faculty.

Students MUST:

    1. Complete and submit the Immunization Documentation Form
      • Student must provide the official English translation of the health, medical and/or immunization records as well as obtain physicians or Doctor’s signature BEFORE submitting to Health Center.
      • Scan/email the completed form(s) to the University.  Send completed forms or documentations to
    2. Complete the Health History form.  Student may need parents to help complete the form.  Submit completed from to email above.  Click HERE for form.
    3. Student completes the Tuberculosis (TB) screening ONLINE form.  Click HERE.  The system requires 9-digit student ID number which begins with G . . . (refer to the admission offer letter.)  The State of Missouri requires ALL new students, staff and professors to complete the TB Risk Assessment form or online.  Please note that TB blood test performed outside of the U.S. is not acceptable.

Student who is not yet 18 years old by August 20, 2018 is required to complete and submit an additional form called the Consent to Treat a Minor form.  The form MUST be signed by parents or legal guardians.  According to the U.S. law, the Health Center cannot provide health care to under-age students without written permission from parents or legal guardian.

The deadline to submit the forms is Wednesday, August 1, 2018 

On arrival on the University campus and during the International Orientation session, new student must participate in the following required health assessment, immunizations or vaccination.  Please note that these required procedures are covered/free to F1/J1 international students on University’s health insurance.

  • Meningococcal vaccine (for students living on-campus residence hall only)
  • TB blood test (for most new students to MWSU)
