COVID-19 Vaccination Incentive Program

Our goal is to provide an engaging, on-campus experience for each student this fall. The campus community can help us do that by getting vaccinated.  COVID-19 vaccines are safe, effective and free, and are available to anyone age 12 and over. To encourage vaccination, we’re pleased to offer Max the Vax, our vaccine incentive program. Students and employees who submit their vaccination record will be entered in drawings to receive prizes, up to a $5,000 cash prize.

How to Enter

  • Submit a copy of your COVID-19 vaccination card or other written documentation through the appropriate secure electronic forms:

  • If you would prefer, you may submit a printed copy. Students may bring their vaccination record to the Esry Health Center, Blum Union 203. Employees may bring their vaccination record to the Human Resources Office, Popplewell 117.
  • The documentation will be entered into your electronic health record or personnel file, for students and employees respectively. All documentation will be treated confidentially.
  • After the university receives your proof of vaccination, you will receive a message confirming receipt and with information about how to claim your free t-shirt.

Grand Prizes

Each grand prize will be awarded to one student and one employee.

Eligibility

  • Participants must have completed a two-dose series of the COVID-19 vaccine, such as the Pfizer or Moderna vaccines, or a single-dose vaccine, such as the Johnson and Johnson vaccine. Any completed COVID-19 vaccine series approved by the World Health Organization will be accepted.
  • Participants must have submitted proof of vaccination electronically or in print by the close of business (4:30 p.m. C.S.T.) on the day prior to the drawing.
  • Participants awarded a weekly prize are not eligible for additional weekly prizes.
  • Participants awarded a grand prize are not eligible for additional grand prizes.
  • Participants who have obtained an accommodation from the ARC (for students) or Human Resources (for employees) regarding their vaccine status are eligible for entry and still must register to be entered into the drawings.
  • Students must be enrolled for at least one credit-bearing course for the fall 2021 semester on the close of business on the day prior to the drawing.
  • Students taking dual credit courses only are not eligible for grand prizes.
  • Student employees and graduate assistants employed by the university are eligible for the student drawings if they are enrolled in at least one credit-bearing class; they are not eligible for the employee drawings.
  • Any cash prize awarded to a student will first be applied to any outstanding balance of their student account prior to the remaining balance being distributed.
  • Employees and emeriti must be employed during the fall 2021 semester.
  • Employees who are enrolled in university courses are only eligible for employee drawings.

Additional Information and Requirements

  • Anyone who receives any prize valued at $600 or more will receive an IRS 1099 form and will be responsible for the taxes on the gift.
  • International students who receive any prize will receive an IRS 1042 form and will be responsible for the taxes on the gift.
  • Some prizes may affect a student’s financial aid package.
  • Prizes are not transferable.
  • Anyone who accepts a prize agrees to allow their name and/or likeness to be announced publicly.
  • Prizes may be subject to certain date restrictions.
