Testing Services Request Form

Please Note:

  • A completed copy of this form will need to be submitted electronically for each exam that is to be administered in the ARC Testing Center. This form is no longer available in paper form. If this form is missing or incomplete, we will not be able to administer the exam.

Student Information

  • The student and instructor will need to work together to determine a mutually agreeable date and time for the student to take this quiz or exam in the ARC Testing Center. Enter the date and time this exam is to be administered in the fields below. Please Note: Testing Services administers exams between the hours of 8:30 a.m. and 3:00 p.m. Exams should be scheduled so that they are completed by 3:00 p.m.

Testing Parameters

  • The information below helps clarify the conditions under which students in the classroom are being examined.
  • How much time have you set aside for students to take this exam in the classroom?
  • We typically provide time extensions of time and a half or double time, based on the student’s needs. For example, when a classroom exam is 60 minutes, students taking their exam in the Testing Center would typically be allowed 90 minutes or 120 minutes according to their Student Information Letter. However, instructors often choose to provide additional testing time to allow students a sufficient amount of time to complete their exams. If you would like to allow this student to have additional time beyond this adjusted time, please indicate below the total amount of time you will allow for this exam:

Completed Exam

  • To protect the integrity of exams administered in the GTC, all completed exams will be kept in a secure location in the ARC. Completed exams may be picked up by the instructor or the exams can be delivered to the instructor by a member of our staff.
