Also, bring with you the names, emails, and dates of birth of your family members.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u2013 OR \u2013<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For the fitness membership,<\/span> <span style=\"font-weight: 400;\">you may choose the monthly payment option through Electronic Funds Transfer (EFT) or a recurring credit or debit card. Your first month\u2019s payment is due upon sign up and the remaining payments will be processed each month for the next 11 months. Stop by Leah Spratt Hall, Room 105, to make a donation and pick up your personalized plate application (these applications can also be picked up at the license bureau).<\/p>\n<p>After you make your donation, Missouri Western will issue you an Emblem Use Authorization Statement.
