Following are the Award Applications for Student Clubs & Organizations Awards Banquet held at 5:30 p.m. on Monday, April 13, 2009.  The applications have changed so please read carefully.

Descriptions and guidelines for the awards are listed on each of the applications attached.

Please note that the application deadline is 4:30 p.m. Monday, March 23, 2009.

Return your application to Student Government Association Blum Student Union 217 or Student Development office Blum Student Union 228.

Applications received past this date will not be accepted.

Application Time Line:

• March 23, 2009 Applications are due
• March 24th – 26th  Selection Committee will review
• March 26th you will be notified of the Selection Committee Decisions via contact information listed in your application.
• April 13, 2009 at 5:30 p.m. you and three members will be present for the banquet announcement.
