Please submit a First Year Requirnment Waiver if you meet any of the above exemption criteria. These forms are available on the Admissions "One Stop" site or by downloading the form from this site.
Immunizations Students wishing to live in the residence halls at Missouri Western State University are required to have up to date vaccinations for measles / mumps / rubella (MMR) tetanus / diphtheria (TD) and meningococcal disease (bacterial meningitis).
