Registration Now Open! Forms Due Back by September 7, 2009!

Registration is now open for the 2009 Heartland Conference on October 2nd- 4th!

The cost of the conference is $100 per delegate. The cost of lodging is included in the registration fee, so no need to make separate reservations for your delegation!

Please download and complete the forms in the right hand box and email them as attachments to heartland2009@missouriwestern.edu. An explaination of these forms is indicated below:

• DELEGATION REGISTRATION: In this form you'll list all the information about each delegate. Delegations can have up to 10 people. Make sure to note any dietary needs or accommodations.
• VEHICLE REGISTRATION: Are you bringing vans, cars, or perhaps traveling by camel? Let us know so we can arrange parking for you on campus.

The following two forms need to be downloaded, completed, signed, and mailed in with your payment ($100 per delegate):

• LIABILITY STATMENT: It's just not Residential Life without a little risk management. Each delegate and advisor will need to read and sign a separate copy of this standard form.
• ALCOHOL AND SUBSTANCE AGREEMENT: In this last form, please print the form, and have each delegate sign and advisor, pledging that they will not consume alcohol or illicit substances while at Heartland 2009.

The first three forms need to be emailed to heartland2009@missouriwestern.edu by September 7, 2009. The last two forms, as well as the check made out to "Missouri Western State University," need to be mailed by September 7, 2009. Checks and forms should be mailed to the address:
