Program of Study Forms

Please consult the Registrar's Web Page for Current Program of Study Forms. Students are required to complete a Program of Study Form during their first semester as a degree seeking graduate student. Without a Program of Study form on file, graduate courses may not count towards your degree.

Discontinued

This option was discontinued in the Summer of 2012. Please see the new Digital Media degree.
