• Student Name
• Student ID Number
• Student E-mail Address
• Scholarship Title
• Number of Credit Hours Successfully Completed
• Cumulative GPA
• Written Statement (Reason for Appeal)
• Supporting Documentation*

*Appeals that site medical, family related issues, etc. must include official supporting documentation. This documentation may be submitted to:
