Each scholarship appeal must include the following information:
- Student Name
- Student ID Number
- Student E-mail Address
- Scholarship Title
- Number of Credit Hours Successfully Completed
- Cumulative GPA
- Written Statement (Reason for Appeal)
- Supporting Documentation*
*Appeals that site medical, family related issues, etc. must include official supporting documentation. This documentation may be submitted to:
