STATEMENT OF PURPOSE

The purpose of this research will be to evaluate a program within the volunteer program of NorthCare Hospice and Palliative Care. I will be conducting a survey of the patients’ families that received a Lifetime Legacy Video. This will be a mail survey to families in which they will have an opportunity to evaluate the program and give feedback for areas of improvement. I will be conducting interviews with the volunteers that work on the videos. During this interview I will be using a standard set of questions to ask all the volunteers, and their names will be kept confidential in efforts to improve the program. The numbers of surveys I plan to send out are 75, each persons will have an equal chance of being selected from the list of persons who received the video of their loved one.

STATEMENT OF RESEARCH METHODOLOGY

The research methodology implemented in this project will be survey sample. The specific type of survey will be a mail questionnaire. The target of my research will be patient families that have received the Lifetime Legacy Video after the patient has died. The data will be collected through mail surveys and then interpreted through coding the results. I will review the findings with the volunteer coordinator as well as write recommendations. The sample will be collected through random selection on the database that holds the family names that chose to have the video done of their loved one.

ANTICIPATED RISKS AND BENEFITS

The benefits are substantial in this research because there will be evidence of how the video helped people grieve as well as how the videos can be improved. The results will be able to help the volunteers who work on the videos improve weak areas as well as continue to implement the strong or popular aspects of the video. This will result in the agency having the ability to improve the way that they assist bereaving persons. The minimal risk is that not all persons will be included in the survey.

SUBJECT SELECTION

This project has significant importance to NorthCare Hospice in the nature that the recipients of the Lifetime Legacy Video have substantial input that would help improve future videos. The research of this particular project will help me gain knowledge about the importance of volunteer programs in hospice and the different services that they can help provide. The knowledge gained in this research project will be helpful in assessing future clientele that may encounter end of life experience and benefit from volunteer services. The potential subjects will be randomly selected from a list of families who received the video, they will have equal opportunity. There will be demographics produced so that there is no discrimination against race, religion, gender, ethnicity, sexual orientation or creed.

CONFIDENTIALITY

The surveys that will be mailed will have no identifiable information on them. I will not ask the name or gender of any persons filling out the survey. On the paid postage envelope there will be no requirement for the subject to fill in the return address to identify him or herself. When the surveys are evaluated and interpreted there will be foreseeable identification. These surveys will be kept in a locked drawer in the volunteer coordinators office. NorthCare Hospice will destroy the surveys after my research project has been completed and my recommendations have been submitted.
