Web Surveys
To obtain a Web-based survey, follow these
simple steps:

Develop your survey and audience demographics.
Type text/questions in a Microsoft Word document.

Obtain written approval for
your survey:
- Surveys sampling any part of Western’s population
must be approved by the Committee on the Use of Human
Subject in Research.

- Surveys conducted by a professor in his or her own
class must be approved by the professor’s dean
or chair.
- Surveys conducted by a student for class assignment
and only in that class must be approved by the professor.
----Note: If there are any questions about surveying,
they should be addressed to the Committee on the Use
of Human Subject in Research.
- Assessments must be approved by the department chair.
Evaluations of internships, site visits, or another
activity required of a course are considered assessments.

Ritter about building your survey. This needs to
be done at least two weeks before you expect
to have your survey completed. Make sure you
have this information:
- Your contact information
- Approval in writing
- Survey content in a Microsoft Word document
- When will your survey or assessment will be active?
- When will you download the data?
- When will the survey or assessment be removed?
- Schedule time to build it yourself or have a member of
the IMC build it for you. Allow at least one week
for building the survey.
- Bring Word document and written approval to IMC at scheduled
time for the building of your survey.
- Disseminate the survey URL to your audience. This is
NOT done by the IMC.
- Return to the IMC for survey data. The software used
to create the surveys has a reporting feature. Data may
also be downloaded to a tab-delimited file for importing
into a number of spreadsheet and database software applications.
- Remove survey from the Web and archive.
