_____  Obtain and complete Immunization Committment to Comply form              

               (downloadable)

_____ Complete and sign Health History Form (downloadable)

_____ Check the tuberculosis risk assessment on Health History Form and

               obtain screening test if required

_____ Return all forms along with a completed Housing and Dining

               Accommodation Application to:
